Job Description

Position Summary:

Under the direct supervision of the Mastercare RN Supervisor, the CNA is responsible for the care of a registered Mastercare client in the privacy of the client’s own home. Caregiver will assist the client with the tasks of daily living and perform quality, compassionate care within scope of client’s care plan.   All care and tasks provided are authorized by a physician and all care must be documented in the clients care plan.  

Minimum Qualifications and Requirements:

  • Must be at least 18 years of age
  • One (1) year experience in:  Hospital, LTC facility or home care within the last two (2) years
  • Must complete Mastercare’s pre-employment and new hire process
  • Pass a physical examination after being offered employment and health assessment annually thereafter
  • Must be able to read, write and speak English. (Some employees may be hired to care for another Nationality will be required to read, write and speak for the Nationality for which they are hired i.e. Japanese Clients)
  • Must be able to follow direction and accurately report to the Field Supervisor an changes in the clients condition
  • Must be emotionally stable and able to display a caring attitude toward clients and their medical needs
  • Must have available, reliable transportation to and from assignments
  • Must maintain current credentials, including CPR, First Aid, TB Test, Physical, Fingerprinting, In-services

Physical Requirements:

  • Must be physically able to perform the duties of the position, including lifting unspecified weights depending on the assignment requirements, bending, stooping, sitting, standing, associated with a normal home care client
  • Must have sufficient visual acuity with correction to read printed materials
  • Must have hearing acuity to hear spoken communications from the client, supervisors, and co-workers
  • Must be able to lift up to 50 lbs. and exert up to 50lbs of force constantly to move required objects consistent with the position.

Work Environment:

  • Employees will be scheduled to meet the needs of Mastercare Clients within their home environments
  • Working with clients in their homes, employees could be exposed to blood borne pathogens and body fluids
  • Employees must utilize personal protective equipment when providing personal care where there is risk to exposure to blood borne pathogens and body fluids
  • Employees may be subject to unrestrained pets, clutter, or poor lighting and performing duties in homes with no air conditioning/ventilation

Essential Duties:

  • Knowledgeable of the individualized Care Plan for clients, and provides support to clients according to their Care Plan
  • Documents services provided in an accurate and timely manner in accordance with agency policy
  • Report changes in the client’s condition or behavior to the RN Supervisor in a timely manner
  • Notifies Nursing Supervisor when clarification or education of a task is required
  • Fully understands all aspects of the client’s rights, including the right to be free of restraints and free of abuse. Is responsible for promptly reporting to the RN Supervisor or management incidents or evidence of client abuse or violation of client’s rights
  • Attends to the individual needs of clients, which may include assistance with grooming, bathing, oral hygiene, meal preparations, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, communicating or other needs in keeping with the client’s care requirements
  • Assists in maintaining a safe, clean, and healthy environment through light housekeeping including changing bed linens, dusting and vacuuming, cleaning kitchen and bathroom, and laundry
  • Performing other housekeeping tasks as indicated in the Care Plan
  • Assists in grocery shopping as needed
  • Assists and escorts to and from appointments / errands
  • Assists and escorts to recreational events & outings. Participates in activities and functions as directed
  • Takes client’s vital signs and recording input and output
  • Communicates and interacts effectively and tactfully with clients, families, supervisor and other employees
  • Work effectively as a team contributor on all assignments by providing specific information and observations of the clients’ needs and preferences
  • Submits timecards/ activity sheet in a timely manner (i.e. weekly)
  • Implements appropriate safety and infection control practices
  • Maintain patient confidentiality at all times as required by HIPAA regulations
  • Other duties as necessary or required

CNA/PA2 are not Allowed to:  

  • Perform tasks not on the Care Plan
  • Perform any skilled nursing including the administering of medication not formally delegated by the RN Supervisor
  • Cut finger and toe nails of any client
  • Give medical or legal advice
  • Provide service for family members of the client
  • Handle client’s checkbook or finances
  • Accept gifts or gratuities; or payment of services
  • Eat client’s food
  • Do household repairs for client
  • Do yard work for client
  • Bring guests to a client’s home
  • Smoke while on shift in a client’s home
  • Use client’s phone for personal calls; text or talk on the phone while on shift
  • Transport (i.e. drive) clients without prior approval from the office
  • Change or cancel shift by notifying the client; always contact the Staffing Coordinator or Mastercare Office.
